# Patient Portal — User Guide

> For your security, after 5 consecutive failed login attempts, your account will be temporarily locked. Contact your care team to unlock it.

## Getting Started

### Creating Your Account

1. Navigate to your organization's Patient Portal at `/portal/register`
2. Enter your **email address** (must match your patient record), create a **password**, and accept the Terms of Service
3. Check your email for a verification link and click it to activate your account
4. You'll be redirected to the portal dashboard

### Logging In

1. Go to `/portal/login`
2. Enter your email and password
3. If MFA is enabled, enter the 6-digit code from your authenticator app
4. You'll land on your **Portal Dashboard**

### Account Lockout

For your security, after **5 consecutive failed login attempts**, your account will be temporarily locked. Contact your care team to unlock it.

## Portal Features

### Dashboard

Your home screen shows quick-access cards to all portal features: Appointments, Forms, Billing, Messages, My Info, and Security settings.

### Health Records

View a read-only summary of your clinical information:

* **Clinical Summary:** Your chart status and chart number
* **Active Medications:** Current medications including dosage and frequency
* **Treatment Goals:** Your treatment objectives in your own words, with progress tracking

> **Note:** Some information may be restricted due to consent requirements. If you see a notice about restricted records, contact your care team.

### Appointments

* **View Upcoming:** See all your scheduled appointments with date, time, provider, and type
* **View Past:** Review your appointment history
* **Cancel:** Cancel a scheduled appointment (select the appointment and confirm cancellation)

### Forms

Complete intake forms, consent documents, and questionnaires assigned to you by your care team. Submitted forms are sent directly to your provider's office.

### My Info (Demographics)

* View your current demographic information on file (name, address, phone, etc.)
* **Request an Update:** If any information has changed, submit an update request. Your care team will review and apply the changes.

### Billing

* **View Balance:** See your current outstanding balance
* **Payment History:** Review past payments and their dates

### Messages

Securely communicate with your care team:

* **View Conversations:** See all message threads
* **Send a Message:** Reply to existing threads or start a new conversation
* All messages are encrypted and HIPAA-compliant

## Security Settings

### Multi-Factor Authentication (MFA)

We strongly recommend enabling MFA for additional account security:

1. Go to **Security** from the portal menu
2. Click **Enroll in MFA**
3. Scan the QR code with an authenticator app (Google Authenticator, Authy, etc.)
4. Enter the 6-digit verification code to confirm enrollment
5. **Save your backup codes** — these are shown only once and can be used if you lose access to your authenticator

### Signing Out

Click **Sign Out** from the menu to securely end your session.

## Proxy / Authorized Representative Access

If you are an authorized representative (parent, legal guardian, or healthcare decision maker) for another patient, you may see their information in the portal. Your access is:

* Granted by the care team (not self-service)
* Time-limited — proxy access has an expiration date
* Scoped — you can only see the linked patient's data

Contact your care team to request or modify proxy access.
